What Insurance Typically Covers at Jersey Family Health
Most major insurance plans cover a broad range of primary care services at Jersey Family Health, often at little or no out-of-pocket cost when using an in-network provider. Below is a general guide to what is typically covered. Coverage specifics vary by plan, and we verify your benefits before every appointment.
Usually Covered at No Cost (Preventive Benefits)
Under most ACA-compliant insurance plans, the following preventive services are covered at 100% when provided by an in-network physician, meaning no copay, no deductible:
- Annual physical exam and preventive care visit
- Well woman exam including pelvic exam and clinical breast exam
- Pap smear and HPV co-testing on recommended schedule
- Routine preventive blood work — CBC, CMP, lipid panel, HbA1c, vitamin D
- Recommended vaccinations — flu, Tdap, pneumococcal, shingles, HPV, COVID-19 and others
- Blood pressure, cholesterol, and diabetes screening
- Depression and mental health screening
- HIV and STI screening for eligible patients
- Colorectal cancer screening referral coordination
- Mammogram referrals as part of preventive screening
Typically Covered Subject to Copay or Deductible
The following services are generally covered by insurance but may require a copay, deductible contribution, or co-insurance depending on your plan:
- Sick visits for acute illness — infections, injuries, respiratory conditions, skin concerns
- Chronic disease management visits — diabetes, hypertension, thyroid, cardiovascular disease
- Follow-up visits after a preventive exam where a new condition is diagnosed or treated
- Diagnostic lab work ordered for a specific symptom or condition
- Specialty lab panels — hormone testing, advanced cardiovascular markers, nutritional deficiencies
- Mental health and behavioral health consultations
- Minor in-office procedures — joint injections, skin lesion removal, wound care
- Telehealth consultations — covered by most major plans, including Medicare and Medicaid
What Insurance Typically Does Not Cover
Not everything falls within insurance coverage and we believe you should know what to expect before your visit. The following services are generally not covered by primary care insurance benefits and may require separate payment, specialist insurance coverage, or prior authorization.
Jersey Family Health Weight Loss Program – this is a comprehensive 6 month program and not covered under insurance
Jersey Family Health Women’ s Perimenopause and Menopause consultation: this is a comprehensive 6 month program
Specialist and Hospital Services
- Specialist consultations — we provide referrals, but specialist visits are billed separately under your specialist’s coverage
- Hospital admissions and inpatient care — covered by your hospital insurance benefit, not primary care
- Emergency room visits — billed separately under your emergency benefit
- Major surgeries and procedures performed outside our clinic
Imaging and Advanced Diagnostics
- MRI, CT scans, and X-rays — ordered and referred by us, billed through imaging facility
- Mammograms — we coordinate referrals, imaging is billed at the mammography facility
Cosmetic and Non-Medical Services
- Cosmetic procedures — not covered by insurance
- Non-medically indicated lab panels or screenings
- Certain travel vaccines not deemed medically necessary by your plan
Prescription Medications
Prescription medications are billed through your pharmacy benefit, not your primary care benefit. Coverage varies significantly by plan, formulary, and medication tier. We help patients access prescription assistance programs where available, and DPC members receive wholesale pricing on select medications.

Frequently Asked Questions - Insurance
Q1: What does 'In Process' mean for insurance status?
A plan marked ‘In Process’ means we are actively completing credentialing with that insurer. We may be able to see you as an out-of-network provider in the interim, or we can discuss our DPC membership as an alternative. Contact our office for the current status of any plan marked In Process.
Q2: Do I need a referral to see you?
Most insurance plans do not require a referral to see a primary care physician. If your plan requires a Primary Care Physician (PCP) designation, you may need to update your PCP with your insurer before your first visit. Call our office and we will walk you through what your plan requires..
Q3: How do I verify my benefits?
We verify your insurance benefits before your appointment and communicate any expected out-of-pocket costs upfront. You can also call the member services number on the back of your insurance card to confirm in-network status and benefit details. Do not hesitate to call our office if you have questions — we are happy to help you understand your coverage.
Q4: Do you offer self-pay options?
Yes. Patients without insurance or with out-of-network plans can access care through our Direct Primary Care (DPC) membership — a simple monthly fee that covers comprehensive primary care with no per-visit charges. We also offer transparent self-pay pricing for specific services. Contact our office at (732) 250-9310 or visit our Pricing page for details.
